Internal/Family Medicine

At Neighborhood Health Center, we take a team-based approach to care that includes doctors, nurse practitioners, physician assistants and medical support staff to promote overall health and wellness. Services include:

Annual physicals
Chronic disease and acute illness management (hypertension, diabetes care, etc.)
Routine shots and immunizations (scheduled vaccinations including flu shots, etc.)
Medication Assisted Treatment (MAT) for substance use
Medication/prescription management, instruction and education
Pre-op evaluation and clearance
Routine lab testing
Sick care
Smoking cessation
Specialist referrals
Testing and treatment of Sexually Transmitted Diseases (STDs)
Total collaborative care with dental, nutrition and behavioral health services available on-site
Workers Comp/No Fault visits for established Neighborhood Health Center patients
