In 2019, an innovative partnership formed between Say Yes Buffalo, Planned Parenthood of Central and Western New York, Buffalo Public Schools, The John R. Oishei Foundation and Community Foundation for Greater Buffalo to extend Neighborhood Health Center’s reach beyond the walls of its health centers, and into the schools and lives of some of our most promised future leaders. Acting as an extension clinic for Buffalo Public School students, Neighborhood’s Health in Motion mobile unit addresses the overall well-being and success of students' primary care needs.
The mobile health unit is staffed by a Neighborhood medical provider, nurse and community health worker to provide:
Well Child Checks & Sick Visits
Immunizations & Preventive Screenings
Physical Fitness Certification for working papers (AT-16 form)
Physical Exams for sports & extracurricular activities
Health Education & Risk Prevention Counseling
Referral Assistance to Community Resources
Referral for Specialty and Treatment Providers
Health Coaching and Vital Sign Readings
