Just because someone is experiencing homelessness doesn't mean that person has to go without quality healthcare.
Neighborhood Health Center participated in the annual Project Homeless Connect event on Thursday, September 28, 2023 at the Buffalo Convention Center. This event is part of a national one-day, one-stop event aimed at helping people connect to housing assistance, healthcare and dental services, utility resources, and much more.
Community health workers from Neighborhood were there to connect people with healthcare appointments at Neighborhood Health Center, offer support with insurance enrollments, health screenings and other social supports.
The WNY Coalition for the Homeless coordinates this event each year with a goal of connecting people who are experiencing homelessness, or at risk for homelessness, to community resources and medical services. As an established Healthcare for the Homeless (HCH) provider since 2012, Neighborhood’s federally funded HCH program provides access to comprehensive primary care, oral health, mental health and substance use services at no cost to the qualifying patient.
If you or someone you know is living on the street, shelter, transitional housing, staying with friends or family temporarily, in a permanent supportive housing facility, or has experienced any of the arrangements above in the last year and is in need of primary and preventive care (and/or in need of a referral to supportive services), contact our HCH team at 716-242-8615 or stop by one of our offices.
