We accept most health insurance plans including Medicaid and Medicare. Regardless of your insurance status, if you have concerns regarding payment and coverage, our Sliding Fee program can help reduce your charges for services based on your household’s size and income.
Before your first visit, please arrive at least 15 minutes prior to your appointment to allow time to fill out any necessary forms.
