Neighborhood Health Center has earned Patient-Centered Medical Home recognition from the National Committee for Quality Assurance (NCQA).

What is a Patient-Centered Medical Home?
A Patient-Centered Medical Home is a model for delivering primary care that puts patients at the center of their healthcare.

Rather than thinking of healthcare as a series of separate appointments or services, the PCMH model emphasizes a coordinated, team-based approach. Healthcare professionals work together—and with patients—to make sure care is connected and responsive to each person's needs.

The model emphasizes:

• Patient-centered care: Listening to patients and involving them in decisions about their health.
• Team-based care: Bringing together providers and other members of the care team to address patients' health needs.
• Coordinated care: Helping patients navigate the healthcare system and coordinating care when they need services from multiple providers.
• Access to care: Working to make healthcare available and convenient for patients.
• Quality improvement: Continuously evaluating and improving the way care is delivered.

NCQA, which administers one of the nation's most widely adopted PCMH recognition programs, notes that the model is associated with improved quality and patient experience, better management of chronic conditions, greater staff satisfaction, and lower health care costs.

PCMH recognition reflects Neighborhood’s commitment to providing high-quality, coordinated care through a team-based approach. Our providers and care teams work together to understand patients' unique needs, coordinate care and support overall health and wellbeing.
