Our North Buffalo site at 2497 Delaware Ave. is welcoming patients
Neighborhood Health Center Delaware, at 2497 Delaware Avenue in North Buffalo, is officially open!
Neighborhood Health Center purchased the site, a vacant former urgent care facility, in late 2025 and renovated it so it could become the organization’s seventh location in Western New York. Neighborhood expanded to the new site because its four current sites in the city of Buffalo are at or near capacity, and Neighborhood’s community needs analysis determined there is a need in the area for high quality primary care that welcomes everyone.
“There are many people in this part of the city who don’t currently have a primary care provider,” says Joanne Haefner, Neighborhood Health Center President and CEO. “We’ve proven that people have better health outcomes when they get patient-centered primary care and preventive services, and they avoid more costly care choices, such as emergency rooms and hospital stays. We want everyone to be able to achieve their highest level of health and wellbeing.”
The Health Resources & Services Administration ranks Neighborhood as the top health center for quality in Western New York, among the top statewide, and in the top 20% nationwide. In patient surveys, Neighborhood earns a 95% overall satisfaction score.
Neighborhood worked with Colby Development on the purchase of the Delaware Avenue property. Renovations took place over the past eight months. The Delaware Avenue site now provides adult and pediatric primary care, and nutrition and behavioral health counseling.
Neighborhood is committed to delivering the highest level of care. New patients are welcome at the Delaware Avenue site and our six other sites across Western New York, with or without insurance. Please call for an appointment: 716-875-2904. Those who are uninsured or underinsured can apply for Neighborhood’s sliding fee program, which is based on income and household size.
